It isn't *technically* referred to as a form, but Lokix's Pokédex entry specifically mentions that it has a "Showdown Mode" which is never shown.

Type: Null was introduced in Alola and, like Arceus, has a form for each type.

There are 11 *total* Ultra Beasts, but UB-02 & UB-04 differ depending on version. Stakataka/Blacephalon are also a version exclusive pair.
