Tender breasts, fatigue, and bowel distress are all unfortunately normal period symptoms. 

What symptoms are NOT normal period symptoms?

Yes! With all her abnormal symptoms she should go see a doctor! 

Should Polly go see her doctor?

Uterus didelphys is a double uterus and usually has no symptoms. Dark masses within the ovary are usually a sign of ovarian endometriosis. A baby would be unlikely as she is not having other symptoms that would indicate pregnancy. 

What pathologically would they see on the ultrasound?

Nothing is wrong so no tests need to be ordered

What tests would be performed?

Eating high fat foods is not recommended for anyone! While changing your underwear regularly is a good thing, it will not help with symptoms of PCOS.

What treatments would they recommended?

Ovulation stimulating medications would be used if she was trying to get pregnant! Testosterone would not be used as it may make symptoms worse.

What medications might be prescribed?

Birth control would prevent pregnancy, testosterones would make PCOS worse, and anti-androgens can be used to reduce poor outcomes and likely be continued.
